Repeat prescriptions
  You can request a repeat prescription by completing and submitting an online form. First you will need to read and accept our terms and conditions. Click on the heading above.

How to register

Our Practice list is open to people who live in the Grangetown, Riverside, Butetown, Atlantic Wharf and Cardiff Bay areas.

Please ring the Surgery or call into reception when the Surgery is open to enquire about registration.

We register patients who live in our Practice area which is: Leckwith Road, Wellington Street, Cowbridge Road East, Castle Street, Queen Street, Churchill Way, Bute Terrace, Bute Street, Herbert Street, Tyndall Street, Schooner Way, Bute Street, New George Street, Stuart Butetown Link, Grangetown Link(A4232).

You can register by:

  • bringing along your NHS medical card or
  • birth certificate, passport or photo driver's licence, and completing a form available from reception.

 

You will be given a new patient pack.

Appointments

The Practice operates an appointment-only system. Morning appointments are available for people who need to be seen that day and can be booked from 8.30am. Once all the appointments are taken further requests to be seen that day may be referred to the doctor on-call.

Afternoon appointments can usually be booked up to 2 weeks ahead.

The Practice makes every effort to ensure that those patients who say they are in urgent medical need are able to consult with a member of our clinical staff. The Practice endeavours to ensure that appointments are offered at times convenient to our patients, with a practitioner of their choice, where possible and that consultations are available with female members of staff. We also offer telephone advice. We have male and female chaperoning facility.

Calls made outside Surgery hours will be forwarded to the company contracted by Cardiff Local Health Board to provide out-of-hours cover, - please telephone the Surgery number for further details.

Please contact us if you think you need a home visit. All requests for a home visit are screened by the doctor on-call.

All clinics operate by appointment, unless stated.

Antenatal Care Routine antenatal check-ups by our qualified midwife. Wednesday afernoon
Mother and Baby Vaccinations for all children up to age 5 and well-baby clinic. Baby-weighing drop-in 10.15-11AM Thursday mornings
Vaccinations Travel vaccinations may be available. Please enquire at Reception.

We run a flu jab clinic in October. We also offer vaccinations for swine flu.
Ask at reception for further information.
 
Asthma Regular check-ups and advice from practice nurses on lifestyle, inhaler techniques and medication are available during surgery times.  
Chronic Disease Managemment Asthma, diabetes.  
Anticoagulation Monitoring For patients on warfarin. Tuesday afternoon
Phlebotomy Blood tests.

Tuesday afternoon Wednesday morning

 

 

Repeat prescriptions

You can order your prescription from Saltmead Medical Centre by any of the following mehtods:

  • by handing in your request at the reception desk
  • by placing it in the prescription request box in our waiting area
  • by posting through the letter-box on the front of the Surgery building at any time
  • by posting your request to the Surgery
  • by faxing your request to the Surgery - see Surgery contact details
  • by emailing your request (see separate section of this Website).

Tick the items you need on the right-hand section of your previous prescription form. Your prescription will be ready for collection 48 hours later. Please allow for the fact that Saltmead Medical Centre is only open Monday to Friday when ordering your prescription.

We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.

Sick certificates

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. You can collect an SC2 self-certificate from the Surgery for shorter periods of illness.

Test results

Please telephone or call in to request the results of your test(s).

To maintain confidentiality the results of tests will only be given to the patient who had the test, unless other arrangements have been agreed in writing beforehand.

Non-NHS services

The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. We are an accredited yellow fever centre. For advice on our fees for these services, please ask at reception.
